Enrollment Paperwork

    • Principal’s Recommendation Form

    • Math and English Teacher Recommendation Form

    • Student Interview Form

    • Parent Interview Form

    • Emergency Information Contact Form

    • Medical Form

    • District Home Language Survey Form

      Documents:

    • Report Cards - Present and 2 years prior

    • Immunization record (including T-DAP Booster required by the State of California)

    • Covid19-Vaccination Proof

    • Copy of Birth Certificate

    • Copy of Mexican Passport (If I-20 is required)

  • Enrollment paperwork MUST be turned in at the time of Registration.

    After taking Placement Exam.

  • We will gladly provide you with paperwork at the school office.
